In this region (Alicante) we are billed every 2 months, you must pay by Direct Debit, yes all properties are metered and the costs vary by region/water company. The tariffs are based on levels of consumption and vary if you go over a certain consumption and include standing charges.

Not only are all properties on mains water metered but even legal wells are licensed to a limit of so many cubic metres a year.
